Small Scale Mine Permit Forms

The following forms/applications are all available for downloading as either a fillable PDF or a PDF that can be completed by hand:

  • Downloadable forms are available in a PDF format and may be printed out and completed by hand.
  • Fillable forms are available in a PDF format and provide you either one or more of the following options:
    • complete the form and then print and mail the form, or
    • complete the form and submit the form electronically
IMPORTANT: It is strongly recommended that fillable PDF forms be downloaded to your computer and completed using Adobe Reader. Fillable PDF forms can no longer be reliably completed in your web browser since many browsers have their own proprietary PDF viewers which DO NOT work correctly with fillable PDF documents.
